Parent/Guardian Waiver and Consent Agreement
To ensure the safety and enjoyment of all participants, we require that a waiver and consent form be signed by a parent or legal guardian for all children attending our events and activities. By signing this form, parents/guardians acknowledge and agree to the following:
- The child is in good health and able to participate in the activities.
- The parent/guardian understands and accepts the risks associated with the event.
- In case of an emergency, the parent/guardian gives consent for medical treatment if necessary.
- The parent/guardian agrees to adhere to all event rules and guidelines set by our team.
